Fees and Insurance:

Payment is expected at the time of service.

Individual providers set their own fees and participate in different insurance plans. Please call our office for details.

We have Preferred Providers who are “in-network” with Cigna, Health First, Magellan, Medicare, Tricare, and Value Options. Fees (co-pays and deductibles) are based on benefits defined by individual insurance plans and vary considerably. Please check with your plan to determine your out-of-pocket expenses.

We are happy to provide necessary paperwork for clients to submit for reimbursement from other insurance plans with out-of-network benefits.

Self-pay rates range from $60-$175, depending on services provided.

We accept Cash, Check, Debit, Master Card/Visa/Discover/American Express and HSA cards.
